Overview
Are you a dedicated nurse ready to make a meaningful impact in rural and regional communities? Join Country Health Connect as a Community Health Nurse (Registered Nurse) and help deliver quality, person-centred care across the Limestone Coast region.
In this rewarding role, you’ll be able to work across multiple Country Health Connect sites in the Limestone Coast, supporting clients in their homes and communities. You’ll assess patient needs, plan and coordinate services, and provide expert nursing care that improves lives. Working in a flexible, multidisciplinary model, you’ll partner with consumers, communities and fellow health professionals to deliver compassionate care where it’s needed most.
Opportunities exist to extend your skills in the Immunisation Program, Wound Care and Continence services.
What You'll Do:
As a valued member of Country Health Connect, you'll be instrumental in delivering high-quality primary health care to clients across the community. Your role will include:
- Assessing and planning patient care to ensure the highest standards.
- Collaborating with other healthcare professionals and community organisations.
- Implementing and coordinating services tailored to each patient’s needs.
- Using your problem-solving, negotiation, and communication skills to drive positive health outcomes.
- Working with diverse groups, including Aboriginal communities, in a culturally sensitive manner.
What We're Looking For:
- Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate
- Ideally, you have experience in community health
- An ability to work within a multidisciplinary team.
- The ability to engage with patients in a culturally appropriate way is a must, and you’ll have a flexible, innovative approach to problem-solving.
- A desire to work towards becoming an Authorised Immunisation Nurse
Country Health Connect focuses on the delivery of exceptional care to those living within our communities who require some support to remain living independently or reside in one of our residential aged care facilities.
