We are seeking passionate and motivated Social Workers (AHP1 or AHP2) to join our Rural Generalist Team within Country Health Connect. Whether you are an early career clinician or have some experience, this role offers the opportunity to develop broad skills across a variety of clinical areas while making a real difference in regional communities.
About the Role
As a Rural Generalist Social Worker, you will work across a range of settings including hospital and community-based services, supporting individuals and families across the lifespan.
This is a varied and flexible role where no two days are the same. You will be part of a multidisciplinary team delivering person-centred care, with opportunities to build capability across multiple service areas.
The position is offered at AHP1 or AHP2 level, depending on experience and clinical capability.
Key Responsibilities
- Provide psychosocial assessment, support and intervention for clients and families
- Deliver care across a mix of acute, subacute and community settings
- Support safe discharge planning and continuity of care
- Work collaboratively within a multidisciplinary team environment
- Link clients with appropriate community, health and social supports
- Maintain accurate documentation and contribute to clinical reporting
- Participate in quality improvement and service development activities
About You
For AHP1 Applicants:
- Early career Social Worker or recent graduate
- Demonstrated ability to apply foundational clinical knowledge in practice
- Willingness to learn and develop across a broad generalist caseload
- Strong communication and teamwork skills
For AHP2 Applicants:
- Experience in Social Work practice within health, community or related settings
- Ability to manage a complex and varied caseload with increasing independence
- Demonstrated clinical reasoning and problem-solving skills
- Experience working within multidisciplinary teams
For All Applicants:
- Qualification in Social Work and eligibility for AASW membership
- Strong interpersonal, communication and organisational skills
- Commitment to person-centred and culturally safe care
- Ability to work both independently and collaboratively
- Current driver’s licence (required for regional service delivery)
