SALHN is seeking Social Workers to join our multidisciplinary teams, in multiple AHP1 positions within Rehabilitation, Aged and Palliative Care. About the role: 

Reporting to the Director, this role is responsible for conducting comprehensive psychosocial assessments, provide counselling support to patients and families, and collaborate with multidisciplinary teams to ensure holistic healthcare delivery.

Duties for the role will include:

• Responding to referrals and contributing to the formulation of plans to assist clients and their carers in rehabilitation, discharge from hospital and reintegration into the community,
• Social assessments and planning and in conjunction with the multidisciplinary team deliver individual and group interventions,
• Providing client and family members with information on rights, benefits and available services including contact with appropriate community resources and support groups to assist the social integration,
• Assisting clients and their carers to adjust to major lifestyle changes including resolving issues of grief and loss

About you:

• A degree and or post graduate qualification which gives eligibility for full membership of the Australian Association of Social Workers,
• Ability to apply a wide range of social work methods,
• Ability to communicate with a wide range of client and professional groups,
• Self-motivation, initiative and perseverance,
• A tolerant and non-judgemental approach to all staff and clients
