Join the Eyre and Far North Local Health Network (EFNLHN) as a Community Support Worker and be part of a transformative care model that puts consumers at the heart of decision-making.

In this rewarding role, you will support individuals who are aged, frail, or disabled by providing essential services such as personal care, meal preparation, cleaning, transport, and social support. Through our Consumer Directed Care model, you will work directly with consumers to help them maintain independence and make choices about their care.

Key Responsibilities:

  • Provide personal care, cleaning, meal preparation, and transport
  • Work with consumers, families, and health professionals to create and maintain personalised care plans
  • Offer emotional and social support to improve well-being and independence
  • Support consumers to meet their goals and preferences in a collaborative manner

What we’re looking for:

  • Compassionate, empathetic, and flexible approach to care
  • Ability to work autonomously and as part of a team
  • Strong communication and interpersonal skills
  • Previous experience in aged care, disability, or community services is preferred
