About the Role

As a Social Worker, you will be part of a welcoming Integrated Care team delivering person centred support beyond the hospital walls. You will provide discipline specific assessment and intervention within a multi-disciplinary team for people with complex care needs including vulnerable consumers, those on a trajectory to the Emergency Department, frequent hospital presenters or those at risk of avoidable hospital re-admission. You will play a vital role in improving the function, recovery and wellbeing of consumers at the interface between hospital and community.

Key duties
•  Deliver culturally responsive assessments and care
• Coordinate care across hospital and community
• Support hospital avoidance and safe discharge
• Advocate a home first approach
• Work across CALHN sites as needed
• Partner with clinics and ED pathways
• Use telehealth and in person care
• Contribute to a 7 day service model
