WHAT ARE WE OFFERING?  

We offer a rewarding opportunity to make a meaningful impact within the Aged Care Support at Home team, supporting older people to live independently through quality, person-centred care. In this role, you will coordinate flexible services, develop care plans in partnership with clients, and lead a team of community support workers to deliver timely and appropriate care. You will work in a collaborative environment, building strong relationships with clients, families, and service providers, while also developing your skills in care coordination, leadership, and continuous service improvement within a consumer-directed care model.

WHO ARE WE LOOKING FOR?  

  • Do you have experience coordinating client services in aged care or a community setting, with a strong focus on person-centred or consumer-directed care?
  • Are you confident conducting home-based assessments, developing care plans, and prioritising referrals to meet client needs effectively?
  • Can you supervise, support, and motivate community-based staff while managing rosters and responding to changing client requirements?
  • Do you have strong communication and interpersonal skills to build relationships with clients, families, and a range of stakeholders from diverse backgrounds?
  • Are you highly organised with the ability to manage competing priorities, maintain accurate records, and ensure compliance with relevant guidelines and reporting requirements?

 HOW WILL YOU CONTRIBUTE?  

You will……….  

  • Deliver high-quality, person-centred care coordination that supports older people to remain independent in their own homes
  • Undertake assessments, develop care plans, and manage referrals to ensure timely and appropriate service delivery
  • Lead and support community support workers, ensuring effective rostering, communication, and quality care outcomes
  • Build strong relationships with clients, families, and service providers to ensure the right care is delivered at the right time
  • Maintain accurate documentation, manage data, and contribute to continuous improvement and compliance activities
