About the Role:
Older adults living in Adelaide's northern suburbs deserve care that keeps them connected to their homes and communities. As a Clinical Nurse in Community Geriatrics with the Northern Adelaide Local Health Network (NALHN), you'll be at the centre of that work — bringing specialist assessment and evidence-based care to patients where it matters most.
What You'll Be DoingBased within NALHN's Modern Primary Health Care unit and working across the Adelaide Metro Northern region, you will contribute to a collaborative, community-focused model of care for older adults with complex and age-related health needs. This is a part-time, short-term contract role through to 19 February 2027. Your key responsibilities will include:
- Conducting comprehensive geriatric assessments and developing patient-centred care plans in community settings
- Coordinating care across multidisciplinary teams, GPs, and community service providers to support safe and appropriate ageing in place
- Monitoring and reviewing patients with chronic, complex, or frail presentations, escalating clinical concerns through established pathways
- Contributing to quality improvement activities and supporting continuity of care across the NALHN primary health network
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (AHPRA)
- Demonstrated clinical experience working with older adults, ideally in a community, aged care, or geriatrics setting
- Strong assessment and clinical reasoning skills, with confidence working autonomously in community environments
- Effective communication and care coordination skills across multidisciplinary teams
- Understanding of SA Health and NALHN care frameworks and standards, or the ability to quickly apply them in practice
- Current driver's licence and willingness to travel across the Adelaide Metro Northern area
To be eligible for this position, you must be Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.

Our Local Health Network Sites and Services:
NALHN provides a range of hospital (inpatient), outpatient, community and home-based services across multiple sites in Adelaide’s north and north-eastern suburbs, including Aboriginal health, mental health, and GP services.
Lyell McEwin Hospital – the major hospital in northern Adelaide for emergency care, complex and multi-day surgery, medicine, obstetric, paediatrics and outpatient services.
Modbury Hospital – a general hospital with emergency care, elective surgery, medicine, outpatient and sub-acute services, rehabilitation, geriatric and palliative care.
