WHAT ARE WE OFFERING?  

Join our dedicated Transitional Care Program team at Mount Barker Hospital, with opportunities available for both a Registered Nurse Level 2 ANUM and an Allied Health Professional AHP2. These permanent part-time roles will directly support consumers as they transition between care settings, promoting independence, wellbeing and positive health outcomes. Working within a collaborative multidisciplinary team, you will play a key role in delivering coordinated, person-centred care while enjoying access to professional development opportunities, a supportive workplace culture and the lifestyle benefits of the Adelaide Hills. Mount Barker, located 40 minutes from Adelaide in the Adelaide Hills, is a perfect blend of natural beauty, local heritage, and gourmet food. • Do you have experience in case management, care coordination or rehabilitation services, with the ability to manage a diverse caseload and achieve positive client outcomes?

• Are you an effective communicator who can navigate complex situations, facilitate referrals and collaborate with a broad range of healthcare and community service providers?

HOW WILL YOU CONTRIBUTE? 

You will….

• Coordinate and deliver high-quality case management services that support consumers to achieve their individual goals and maximise independence.
• Develop, implement and monitor comprehensive care plans, working collaboratively with consumers, carers and multidisciplinary teams to achieve positive outcomes.
• Facilitate timely assessments, referrals and access to appropriate services, ensuring seamless transitions across the healthcare continuum.
• Contribute to quality improvement, clinical governance and evidence-based practice initiatives that enhance service delivery and consumer experience.
• Foster strong professional relationships and provide leadership, guidance and support to colleagues, students and less experienced staff within a collaborative team environment.

WHAT ELSE DO YOU NEED TO KNOW/WHAT IS ESSENTIAL FOR SUCCESS?

• You will need to undergo employment screening, what you require is also on the front page of the Role Description. 
• You will be required to hold registration with AHPRA to practice as an Enrolled Nurse within our organisation.
