About the role

VMCH is seeking a passionate Physiotherapist to join our Home Care team in a full-time, 12‑month contract role.

As part of our Allied Health team within the At Home Aged Care (AHAC) service, you’ll deliver evidence-based physiotherapy services to elderly clients under the Commonwealth Home Support Programme (CHSP) and Home Care Packages (HCP). These roles are pivotal in helping clients remain independent and safe in their homes and communities. 

Ready to learn more about what you will be doing in this rewarding role? 

• Conduct comprehensive assessments and deliver tailored interventions aligned with aged care quality standards
• Collaborate with clients to identify goals and develop care plans
• Provide group-based programs focused on cognition, balance, wellness, and reablement
• Collaborate with multidisciplinary teams including nurses, occupational therapists, and care coordinators
• Maintain accurate documentation and meet billable hour targets
• Travel across aged care planning regions as required

About you 

• Tertiary qualification in Physiotherapy and current AHPRA registration
• Strong clinical assessment and report writing skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively
• Current driver’s licence and willingness to travel across Metropolitan Melbourne
• Valid Police Check and Australian working rights
