About Your Role
Scope has an exciting and unique opportunity for a Service Transition Coordinator to work within our Chief Employment Pathways Officer division.
In this role, you will be responsible for coordinating the transition of clients into Scope’s services; contributing to an experience that empowers our clients to belong and thrive, now and into the future.
You will contribute your knowledge and experience to:
▪ Working collaboratively to continue to improve the journeys and experiences of clients and their support networks (e.g. families).
▪ Identifying, exploring and advocating for opportunities to improve how we onboard clients; with a focus on safety, quality and sustainability.
▪ Coordinating key requirements and activities to facilitate a seamless transition into our services for our clients.
Your Key Focus
▪ Maintain accurate documentation; collating and submitting it to relevant internal and external stakeholders, in alignment with relevant legal, regulatory and policy requirements.
▪ Communicate with stakeholders to clarify expectations, explain funding and services availability and resolve and/or esca late issues.
▪ Share feedback to support the development, implementation and ongoing evolution of Client Acquisition and Retention’s plans and priorities.
▪ Consistently follow relevant client acquisition and retention processes and practices to deliver outcomes.
▪ Consistently follow customer quality and safeguarding policies, systems, practices and controls.
▪ Consistently follow relevant risk management practices; seeking advice where required.
▪ Understand and adapt with organisational change; supporting others to do the same.
▪ Critically reflect to identify and share opportunities for innovation and continuous improvement.
What We Look For
▪ Ability to collaborate with internal and external stakeholders to set expectations and deliver outcomes.
▪ Ability to prioritise and manage at times competing priorities, stakeholders and expectations to deliver outcomes.
▪ Experience (or the ability to quickly acquire) communicating with others, including through different communication modes.
▪ Experience in the disability services and/or a related sector would be advantageous.
▪ Experience in a not-for-profit organisation/s and/or a related sector would be advantageous.
