The Role

The Care Coordinator has clinical responsibility for the ongoing delivery of care to the residents within their allocated area, providing clinical leadership and expertise to the team of Registered Nurses (RNs), Enrolled Nurses (ENs) and Carers ensuring the standard of care delivered meets customer and Amana Living expectations. This is a Part-time position working 45.6 hours per fortnight from our Lady McCusker Home in Duncraig.

The shift pattern is as follows:

Week 1 & 2

Monday – Wednesday: 0830-1645

 

Key responsibilities:

  • Check the site allocation sheet and ensure adequate staffing for the shift and assist with covering any Clinical or Care vacancies.
  • Direct Clinical and care staff in the follow up any other queries, problems, concerns identified during handover.
  • Complete facility rounds: Management by Walking and visual observation – provide feedback & support to staff during these rounds, Commend staff achievements/improvements.
  • Centre presentation/environment check (Cleanliness or rooms and ensuites, correct Falls prevention and management strategies are being maintained throughout 24/7 cycle of care).
  • Provide clinical care oversight as per best practice principles.
  • Liaise with and provide support to the GP and Allied Health Providers during their visits to the facility & follow up as directed with medication management, appointments, and referrals.
  • Ensure hospital discharge recommendations are followed up according to discharge summary.
  • Complete investigation and analysis of Riskman investigations.
  • Full wound audit (Checking wound charts match/ reflect Wound Care Assessments, weekly photo of wounds has been completed)
  • Completion of Key Performance Indicator report (Responsive Behaviours, Falls, Infection, Medications, Nutrition, Restraints, Skin integrity, unplanned hospital admissions weight management)

Leadership Responsibilities include:

  • Managing resident and visitor complaints and de-escalating throughout the shift, escalating when required.
  • Coaching conversations and managing poor performance throughout shift, file noting and escalating to Service Manager as required.
  • Provide mentorship and training to all Clinical & Care staff/ Graduates or student nurses.
  • Carry out Clinical Competencies Trainings as required.
  • Staff Orientation – ensure compliances are completed and assist in the orientation of new staff, casual or agency as required.
  • Coordinate External Services: Hearing, Dental, Optical, SP, Dietician, etc.
  • Coordinate Student Placements.

 

About you

The successful applicant will demonstrate a high level of clinical expertise and currency of practice with proven organisation and time management skills. Ideally you would have a minimum of 5 year as a Registered Nurse with an understanding of Aged Care. You will have the ability to articulate and role model consumer dignity and choice impacting on assessment and service delivery through open consultation with residents and relatives.

 

Essential Criteria:

  • Evidence of COVID-19 and Current 2024 Flu vaccinations per Amana Living policies
  • Ability to obtain a National Police Clearance (within 6-months validity)
  • Successful completion of pre-employment physical and reference checks
  • Registration with AHRPA and a minimum five years’ experience as a Registered Nurse.
  • Available to attend a compulsory 2 Day (Monday & Tuesday) induction at our Rivervale Training Institute
