Job Summary: |
The Department of Social Work at SUNY Downstate Health Sciences is seeking a full-time TH Social Worker 2. The Social Worker is an integral part of the hospital multidisciplinary team. The Social Worker works with the patients, their families and caregivers to assess the patient and family psychosocial and discharge planning needs (relevant to medical treatment). The Social Worker is responsible for the coordination and delivery of care services to children, adolescents, ad their families as well. They educate patients, family and staff on diagnosis, treatment programs and resources available. The Social Worker also conduct psychosocial, emotional, and socioeconomic assessment of children with life threatening diseases (with family members) and collaborate on care plans with evaluation of patient outcomes. They also collaborate with other members of the Pediatric Care team to assure high quality of care. They provide crisis intervention, emotional support, resource information, discharge planning and legal reporting. The Social Worker’s goals are to:
