We are seeking expressions of interest from individuals interested in a career as a Care Facilitator with BaptistCare At Home. Due to continued growth, upcoming opportunities are available across multiple locations, with both full-time and part-time options and flexible work-from-home arrangements.
In this role, you will partner with clients and their families within your local government area to deliver quality care coordination and service planning, supporting clients to live independently at home.

About the Role
This position provides dedicated support across your local government area and nearby communities, with regular travel required. You will work closely with clients and their families to develop individualised care plans, set achievable goals, and connect them with services that enhance their wellbeing.
Your day may include:
- Visiting clients in the community who are referred for in-home support
- Developing and reviewing tailored care plans
- Building strong relationships with families and carers
- Collaborating with multidisciplinary teams to deliver best practice care
- Maintaining accurate documentation using care management system
What You’ll Bring
Required qualifications and experience:
- Bachelor’s Degree or Diploma in Community Services (Service Coordination or Case Management) or equivalent experience
- Experience in aged care or transferable experience in a community-based service environment
- Ability to work independently and as part of a team
- Strong communication, time management, and organisational skills
- Intermediate computer skills, including email and applications
- A valid Australian Driver Licence and a reliable vehicle with minimum Third Party Property Damage Insurance
