The Role

As the Continuity of Care Coordinator, you will play a vital role in ensuring seamless patient journeys across the SA Health System, focusing on Aboriginal children aged 0-4 and their mothers. Your expertise will help provide a responsive liaison service that connects clients and health services, making a meaningful difference in their care.

In this dynamic position, you'll coordinate consultations with consumers and stakeholders to assess service options tailored to individual needs. You'll also engage in vital data collection, document preparation, and reporting, all while contributing to the improvement of health outcomes for Aboriginal families.

The Port Augusta Hospital and Regional Health Service is a large regional hospital that provides a wide range of services including emergency, general and surgical, maternity, outpatient, and outreach services. The hospital has 58 beds including flex and monitored beds.  The Port Augusta Hospital also has chemotherapy services and has a Renal Dialysis Unit and a Paediatric Department, both of which are the largest outside of Adelaide. Our staff work in close partnership with community health services and Aboriginal health providers, to ensure that optimum care is delivered to all areas of the community.
