If so, we invite you to join our dedicated team as a Case Coordinator - Home Care Packages with the Eyre & Far North Local Health Network (EFNLHN) in Port Lincoln/Lower Eyre.

Overview

As a Case Coordinator for Home Care Packages (HCP), you will play a pivotal role in delivering a high-quality Community Care Program. This involves the identification of needs, assessment, implementation, coordination, maintenance, and evaluation of home and community-based programs. Your efforts will ensure that our clients—those who wish to remain in their own homes with some support—receive the best possible care and services.

Key Responsibilities:

  • Program Development: Design and develop community-based programs tailored to the needs of ageing and disabled individuals.
  • Assessment & Coordination: Assess client needs, coordinate services, and implement care plans in collaboration with a multi-disciplinary team.
  • Evaluation & Review: Regularly review and evaluate the effectiveness of programs and services to ensure they meet the required standards and deliver optimal outcomes.
  • Client Support: Provide comprehensive support and guidance to clients, helping them navigate services and resources effectively.
