| This position reports to the Office of Continuous Improvement (OCI) Administrator. This position is a leader within the agency that is responsible for the establishment and maintenance of a lean management system that is embedded within culture of the business, fosters a continuous improvement mindset, and practices psychological safety. The Lean Practitioner plays a critical role in advancing the mission of our social welfare organization by leading initiatives that enhance service delivery, operational efficiency, and client outcomes. Furthermore, this role partners with cross-functional teams to identify, design, and implement sustainable improvements using Lean, Six Sigma, and human-centered design principles. | Under the direction of the Utilization Review (UR) Administrator, this position is responsible for management and oversight of the DFSM Behavioral Health Prior Authorization/Utilization Review (BH PA/UR) of staff and unit processes. Including interaction with Providers, resolution of Provider issues and BH PA/UR reporting. Provide monitoring and technical assistance to ensure compliance with contractual, regulatory, and statutory obligations for the members’ physical/acute care services and behavioral health needs. |

| The Permanency and Youth Services Unit supports transition age youth who are experiencing foster care. The Unit strives to offer youth centered supports and services in order to ensure young people are prepared to live outside of the foster care system.
The Transitional Housing Coordinator will support housing opportunities for young adults in Extended Foster Care. They will work with community partners, contracted providers, DCS Specialists and youth to determine which housing option best meets their needs. They will provide technical assistance to Contracted Provider, as well as oversight and monitoring of the programs to ensure a positive experience for young adults. |

Human Services Program Development Specialist

| The Extended Foster Care (EFC) Consultant provides direct coaching and oversight to EFC Success Coach Program Providers in order to ensure successful implementation of the EFC Success Coach Program. The EFC Consultants will be responsible for group coaching sessions to EFC Success Coaches and their supervisors on how to engage youth in a coach-like way and provide strengths-based case management as needed for youth and young adults participating in the Extended Foster Care. They will organize and facilitate Age of Majority (AOM) meetings for youth in out of state Behavioral Health facilities at 17.5 and older and all become trained trainers of Youth Thrive, and the DCS Specific training. They will track coaches with the contracted providers, and report and monitor the Fidelity of the program model. The EFC Consultants will be experts in Arizona’s Young Adult Program, as well as the BH system with an emphasis on DCS’s Extended Foster Care Program. |
