Are you committed to enhancing the quality of life for individuals in our community? Do you have a passion for coordinating comprehensive care programs? Join us as a Home Care Coordinator for the Community Health Support Program (CHSP) with the Eyre & Far North Local Health Network (EFNLHN).

Role Overview:

As the Home Care Coordinator, you will be instrumental in delivering a high-quality Community Care Program under the CHSP. Your role will encompass identifying client needs, conducting assessments, implementing and coordinating services, and maintaining and evaluating home and community-based programs. You will work within a multi-disciplinary team to develop and manage programs that support ageing and disabled individuals wishing to remain in their own homes with tailored assistance.

Key Responsibilities:

  • Program Management: Oversee the development, implementation, and coordination of community-based care programs under the CHSP framework.
  • Needs Assessment: Conduct comprehensive assessments to identify client needs and determine appropriate services.
  • Service Coordination: Ensure the effective coordination of services and resources to meet client needs and enhance their quality of life.
  • Program Evaluation: Regularly evaluate and review programs to ensure they are effective and aligned with EFNLHN’s standards and client expectations.
  • Team Collaboration: Work closely with a multi-disciplinary team to ensure holistic and integrated care for clients.
