Join SA Health’s Remote Health Monitoring (RHM) Service in a predominantly clinical role delivering high-quality, evidence-based nursing care in a virtual practice setting. As a Clinical Nurse (CN), you will provide comprehensive clinical assessment and individual case management for clients with chronic and complex conditions, using telehealth and digital health technologies to support care closer to home. This role plays a key part in improving patient outcomes, reducing hospital demand, and strengthening continuity of care across regional South Australia.

Key Responsibilities:

  • Provide advanced nursing care and case management for clients with chronic and complex conditions.
  • Deliver care using remote health monitoring and digital health technologies.
  • Support reduced hospital admissions, length of stay, and patient travel.
  • Manage complex cases while maintaining high clinical and governance standards.
  • Contribute to continuous improvement and a collaborative, patient-centred team environment.

Who Are We Looking For?

  • Do you have advanced clinical experience in chronic disease management, ideally in virtual or rural settings?
  • Are you confident using telehealth, digital health technologies, and client databases to deliver care?
  • Do you demonstrate high-level communication, problem-solving, negotiation, and conflict-resolution skills?
  • Are you committed to maintaining clinical standards and contributing to continuous improvement?
