We are seeking a Nurse Practitioner Candidate to join our Community Geriatric Evaluation and Management (GEM) team, delivering advanced nursing care to older consumers in their homes and community settings.

As part of our Community GEM program, you will work alongside experienced healthcare professionals to provide comprehensive care to patients across diverse community settings. This role supports your journey towards full Nurse Practitioner endorsement while contributing to innovative healthcare delivery models that prioritise patient-centred care and health equity.

About the Role

Community GEM is a multidisciplinary team that provides comprehensive in-home geriatric assessment, short-term intervention and case management for clients over 65yrs who are living in the community with complex health needs. The aim of the service is on hospital avoidance and supported discharge from the hospital environment, with the client being linked to appropriate long-term services to support them in ongoing management of their health needs. 

Key Responsibilities

  • Provide advanced nursing care and clinical assessment within community healthcare settings
  • Participate in quality improvement activities and evidence-based practice development
  • Mentor and support junior nursing staff
  • Provide specialist geriatric care in the community, helping older consumers maintain independence and avoid unnecessary hospital admissions.
  • Work collaboratively with a multidisciplinary team to deliver person-centred care tailored to complex needs.
  • Lead innovative care models and contribute to hospital avoidance strategies through education, equipment provision, and referrals.
  • Develop your advanced practice skills under the guidance of an authorised Nurse Practitioner and medical mentors.

About You

To be eligible for appointment to the role, you will need current registration as a Registered Nurse and eligibility to enrolled in a Nurse Practitioner program approved by the Nursing and Midwifery Board of Australia.

You will bring:

  • A minimum of 3 years post-registration experience, with demonstrated competence in geriatric or community nursing
  • Strong leadership, communication and problem solving skills
  • The ability the work independently and collaboratively in a complex and dynamic community setting
  • A commitment to consumer engagement, cultural respect and continuous improvement 

Your empathy, patience, and adaptability will be key to supporting older consumers and their families through complex health journeys.

Lyell McEwin Hospital The major hospital in the north and north-east for emergency care, complex and multi-day surgery, medicine, obstetrics, paediatrics and outpatient services.

Modbury Hospital A general hospital with emergency care, elective surgery, medicine, outpatient and sub-acute services, rehabilitation, geriatric and palliative care.

Mental Health - The Division of Mental Health provides a comprehensive range of public mental health services for youth, adults and older people in northern Adelaide. Services are provided through community health centres and hospitals, and to consumers in their own homes. 

Forensic Mental Health - A statewide service that operates as part of the Division of Mental Health for forensic patients and prisoners. 

Community Sites - Elizabeth GP Plus, Modbury GP Plus and Gilles Plains GP Plus Super Clinics providing community based care.
