Are you an experienced clinician or health professional passionate about leading high-quality community aged care services? Join YNLHN as Team Leader – Healthy Ageing Southern and provide leadership, direction, and operational management across a multidisciplinary team. In this role, you’ll oversee community-based programs for older adults, ensuring services are responsive, safe, and aligned with the Commonwealth Aged Care Standards and YNLHN’s Model of Care.

This position is open across three classification levels, reflecting your professional background and experience:

  • RN/M3: Provides clinical leadership within community aged care nursing services, contributing to operational management, workforce development, and quality care delivery.
  • ASO6: Focuses on operational management, service evaluation, and delivery of in-home and community-based services, including personal care, domestic assistance, social support, and aged care assessments.
  • AHP3: Delivers clinical and professional leadership to allied health staff, ensures service quality, safety, and community-focused outcomes, and contributes to workforce and model-of-care initiatives across YNLHN.

Key Responsibilities:

  • Lead and provide direction to a multidisciplinary team within a reablement-focused community aged care setting.
  • Oversee and manage operational delivery of community aged care programs, including home-based and group services, and aged care assessments.
  • Apply detailed knowledge of Aged Care Standards, Consumer Rights, and Consumer Directed Care to ensure compliance and high-quality service delivery.
  • Contribute to service planning, redesign, and professional development initiatives in response to community and client needs.
  • Monitor, evaluate, and report on service provision, outcomes, and workforce performance.
  • Support the development, mentoring, and guidance of staff across nursing, allied health, and administrative roles.
  • Manage resources effectively, including staffing, rostering, budgets, and equipment allocation.
  • Participate as a member of the Community & Allied Health leadership team, contributing to strategic management and continuous quality improvement.

Each classification level (RN/M3, ASO6, AHP3) has its own essential minimum requirements for educational/vocational qualifications and professional experience. You’ll have demonstrated experience in managing teams, delivering client-focused programs, and ensuring high standards of care and service quality.
