About the role

This is an excellent opportunity for a senior pharmacist to join the CALHN Integrated Care team. Integrated Care encompasses a range of programs, including, but not limited to Urgent Care/Hospital avoidance hubs, Hospital in the Home and community geriatrics programs, working uniquely at the interface of hospital and community, promoting 'home first' care, hospital avoidance and continuity of care.

This role will be predominantly working with the multidisciplinary Hospital in the Home service to contribute significantly to direct patient care in the home and medication safety initiatives with patients and staff. Some work in other Integrated Services may be required.

More broadly you will be part of CALHN Pharmacy with access to the full range of expertise available in our dispensing, electronic medication management, manufacturing, investigational drugs, clinical, and medication governance teams.

Some Integrated Care services operate across 7 days, with access to the multidisciplinary team critical to the success of the model so some weekend work will be expected.

About You

You are a motivated and experienced hospital pharmacist who brings strong clinical expertise, leadership capability and a commitment to delivering high-quality, patient-centred pharmacy services. You will bring:

  • Recognised qualifications and registration, including a Bachelor of Pharmacy (or equivalent) and current registration, or eligibility for registration, with the Pharmacy Board of Australia.

  • Extensive hospital pharmacy experience and professional knowledge, with demonstrated technical competence in pharmacy practice and a comprehensive understanding of relevant standards, codes of practice and service requirements.

  • Strong leadership, communication and teamwork skills, with proven experience supervising, educating and training staff, working effectively within multidisciplinary teams, and providing clear direction to improve service delivery.

  • Highly developed analytical, organisational and problem-solving skills, including the ability to work autonomously, manage competing priorities and deadlines, undertake research and data analysis, and implement practical, quality-focused improvements.
