About the Role

As Heart Failure Nurse Consultant, you’ll lead coordinated, patient-centred care across hospital and community. You’ll manage complex cases, build trusted partnerships, and shape evidence-based pathways that support people to live well with heart failure.

• Lead case management for heart failure patients across care settings
• Coordinate specialist heart failure clinics at RAH and TQEH
• Deliver home, clinic and group education and support
• Link acute, primary and community services for seamless care
• Guide nurses through expert clinical leadership
• Partner with cardiology, allied health and GPs to plan care

About You

You’re a confident cardiac nurse ready to lead, connect and deliver high-quality heart failure care across settings.

• Registered Nurse with AHPRA registration or eligibility
• Strong cardiology practice aligned to professional standards
• Lead teams with clear communication and sound judgement
• Build trust across multidisciplinary and community partners
• Guide staff development and support change in practice
