About the Role

As a Social Worker, you’ll support patients and families to navigate care with confidence. You’ll manage complex psychosocial needs, build trusted links across teams and community, and help create care pathways that improve outcomes across single and multi-site services.

• Assess psychosocial needs and plan timely support
• Deliver counselling, consult and provide education
• Liaise with clinical teams, services and community partners
• Contribute to ward rounds, case meetings and care planning
• Collect data and support quality improvement and research

About You

AHP1
You’re an early-career Social Worker with a strong foundation and a desire to grow. You bring organised practice, clear communication and a collaborative mindset to support patients, families and teams.

• Eligible for full AASW membership via approved degree
• Apply social work theory to assessments and care plans
• Prioritise tasks and deliver timely outcomes
• Communicate clearly with teams, patients and services
• Work safely and flexibly across CALHN services

AHP2
You’re an experienced Social Worker who brings confidence, autonomy and depth of practice. You apply advanced judgement, support others and contribute to service improvement and strong patient outcomes.

• Eligible for full AASW membership via approved degree
• Apply advanced social work theory and evidence in practice
• Manage complex priorities and competing service demands
• Communicate with influence across teams and partners
• Support colleagues and work flexibly across CALHN
