About the role

As a Nurse Consultant (Care of Older People), you will provide expert clinical nursing leadership focused on the specialised care needs of older patients. Collaborating closely with multidisciplinary teams, including ward NUMs, medical staff, and allied health professionals, you will coordinate and manage comprehensive care plans, ensuring timely referrals and seamless discharge planning. Your role will be pivotal in delivering age-appropriate care, promoting best practice, and improving patient outcomes across a variety of clinical settings.

You will also act as a key liaison with external healthcare providers, residential care facilities, and community services, fostering strong relationships to support admission avoidance and ongoing care coordination. By working with local staff, you will enhance aged care knowledge and skills, while leading projects and programs that drive continuous improvement. This role demands accountability, clinical expertise, and the ability to influence care standards to meet the unique needs of older people within our health network.

About you

You are a dedicated and experienced nursing professional passionate about delivering exceptional care and leading positive change. With strong leadership and communication skills, you thrive in multidisciplinary teams and are committed to fostering staff development and consumer engagement.

  • Registered or eligible nurse with current or eligible practising certificate

  • Proven leadership skills with expertise in communication, problem solving and change management

  • Ability to work collaboratively, prioritise tasks and meet deadlines effectively

  • Strong knowledge of healthcare standards, legislation and quality improvement systems
