About the Role

As a Registered Nurse (RN1) within the CALHN Urgent Care Hub (BRIGHT), you will be part of a supportive, community-focused team delivering acute and sub-acute care to people with complex health needs. This innovative same-day hospital avoidance service operates seven days a week, including public holidays, providing rapid multidisciplinary assessment and coordinated care to prevent unnecessary Emergency Department presentations and inpatient admissions.

In this role, you will provide person-centred nursing care through comprehensive assessment, clinical intervention within a clinic-based setting. Working collaboratively within an integrated service model, you will contribute to early discharge planning, restorative care and proactive management of patients who would otherwise require acute hospital services.

You will build strong partnerships across the health system, including General Practitioners, SA Ambulance Service, pathology, pharmacy, acute services and community organisations, ensuring seamless transitions of care and improved patient outcomes.

Duties

• Conduct comprehensive nursing assessments and contribute to care planning
• Deliver safe, evidence-based acute and sub-acute nursing care
• Support coordinated responses that enable hospital avoidance and early discharge
• Collaborate effectively with multidisciplinary team members and external partners
• Participate in a 7-day roster including weekends and public holidays
• Contribute to quality improvement, audit activities and safe clinical practice
• Support the learning and development of students and junior staff as appropriate

About You

You are a confident and compassionate Registered Nurse ready to deliver safe, high quality care within a dynamic integrated service. You bring the clinical capability, judgement and drive to thrive in complex acute and community settings.

• AHPRA registration with current practising certificate
• Strong clinical assessment and care planning skills
• Safe care delivery aligned to NSQHS standards
• Clear communication and team collaboration skills
• Sound knowledge of legislation and quality systems
