About the Role

Join Statewide Aged Care Assessment Service (ACAS) as an Assessor and help older clients access the care they need. Manage assessments, build strong connections, and maintain accurate records while guiding clients through aged care pathways.

  • Conduct needs-based assessments in community and hospital settings

  • Enter accurate data in my Assessor App and My Aged Care systems

  • Advise clients on care options across State and Commonwealth pathways

  • Liaise with clients, carers, health professionals and service providers

  • Support quality, compliance and continuous improvement activities

Qualifications

RN-2

  • Registered as a Nurse with the Nursing and Midwifery Board of Australia.

Must hold a current practising certificate.

  • My Aged Care Learning (MAC Learning) Assessor Certificate or Aged Care Assessment Organisation (clinical) Assessor Certificate.
    • Integrated Assessment Tool (IAT)
    • Triage
    • Delegation

AHP-2

  • A degree or other qualification which entitles registration as a physiotherapist, or as an occupational therapist, or a degree level qualification in the social work discipline enabling eligibility for membership to the AASW or to a standard accepted by the Chief Executive.
  • My Aged Care Statement of Attainment 4 (SoA4)
  • My Aged Care Self-Paced Learning Experience (SPLE)
    • Working appropriately with Aboriginal and Torres Strait Islander Peoples (ATSI)
    • Working with Culturally and Linguistically Diverse People (CALD)
    • Working with Carers and the Care Relationship (Carers)’
