Hours/Period of Appointment

1 x Permanent Shift Worker, working 76 hours per fortnight.

Notwithstanding hours may be negotiated with the successful applicant.

Who are we looking for?

We are seeking a motivated and experienced Clinical Coordinator - Community Health Nursing for Healthwest – Rosebery Community Health Centre. Within the dynamic multidisciplinary team, you will contribute to a strong professional team approach through open communication and a positive supportive environment.

Reporting to the Nurse Unit Manager, you will coordinate the provision of high quality community health nursing and ancillary services to clients on the West Coast. You will provide clinical leadership and support efficient and effective coordination of client care based on national standards and best practice using a collaborative and multidisciplinary framework.

Essential Requirements

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse.
  • Current Driver's Licence.
  • The Head of the State Service has determined that the person nominated for this job is to satisfy a pre employment check before taking up the appointment, on promotion or transfer.  The following checks are to be conducted:
  1. Conviction checks in the following areas: a) Crimes of Violence, b) Sex Related Offences, c) Serious Drug Offences, d) Crimes involving dishonesty 
  2. Identification check
  3. Disciplinary action in previous employment check.

Desirable Requirements

Holds or is working towards appropriate post-graduate qualifications.
