Job description:

Within the Tasmanian Health Service framework a non clinical  Aged Care Assessor conducts Home Support Assessments through the My Aged Care platform. Using the Integrated Assessment Tool (IAT) the assessor identifies and evaluates an older person’s needs, goals and preferences with a focus on wellness, reablement and promoting independence.

The Team:
The Tasmanian Health Service delivers the Aged Care Assessment Service (ACAS) South which provides both clinical and non-clinical assessments as part of the Assessment and Case Management Service. Our dedicated team is committed to delivering high-quality, person-centred care that supports older Tasmanian’s in accessing  appropriate government funded aged care services.

The Role:

Within the Tasmanian Health Service framework a non clinical  Aged Care Assessor conducts Home Support Assessments through the My Aged Care platform. Using the Integrated Assessment Tool (IAT) the assessor identifies and evaluates an older person’s needs, goals and preferences with a focus on wellness, reablement and promoting independence.

You will need:

  • Well-developed understanding of and demonstrated ability to undertake the functions of an Aged Care Assessor including the needs of an older person and the ability to plan and co-ordinate client care independently and operate effectively within a multidisciplinary team. 
  • Demonstrated understanding and knowledge of the needs of older clients and the concept of wellness and reablement and the use of funded and non-funded services to enable people to meet their goals. 
  • Strong organisational skills and ability to meet deadlines.

Details of appointment:

Permanent full time, day worker position, working 76 hours per fortnight, commencing as soon as possible.

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $81,915 to $94,263 per annum. Our Employer 11.5% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Public Sector Unions Wages Agreement 2022

*Registration/licences that are essential requirements of this role must remain current and valid at all times whilst employed in this role and the status of these may be checked at any time during employment. It is the employee’s responsibility to ensure that registration/licences remain current and to advise the Employer if their circumstances change. This includes notifying the Employer if a registration/licence is revoked, cancelled or has its conditions altered

Applicants should note the following criteria are desirable:

  • Current Driver’s Licence
  • Previous experience in case management, aged care or support work care co-ordination and individualised service planning
