Job description:

Following Agency policies, primary health care principles, legal requirements and professional competencies, the Clinical Nurse Consultant - Community Health Nursing West Coast (CNC), in collaboration with the Nurse Unit Manager (NUM) and Director of Nursing (DON), provides clinical leadership and support to the community healthcare team by coordinating the provision of high quality community health nursing, and ancillary services to clients on the West Coast based on national standards and chronic disease best practice principles within a multidisciplinary framework. In consultation with the NUM, proactively direct, lead, and participate in the development of an education and research culture. Support the NUM in developing, implementing, and evaluating clinical practice standards, including monitoring clinical competencies of the health care team, and actively participating in the professional development of individual team members. As a senior member of the healthcare team, support the NUM in a broad range of administrative, service delivery, and business planning issues relating to the provision of community nursing, adult day centre, home, and community care.

The Role

The Clinical Nurse Consultant - Community Health Nursing West Coast (CNC - CHNWC) is the clinical lead of the area and works collaboratively with nursing, medical and allied health clinicians in the communities of Queenstown, Strahan, Zeehan and Rosebery (based in Rosebery, travel in the region required):

  • Lead and promote the efficient and effective provision of community-based care based on best practice clinical and organisational principles, specifically in alignment with the Primary and Community Health Care Standards, National Primary Health Care Standards, and Aged Care Quality Standards.
  • Maintain a collaborative partnership with internal and external stakeholders, including relevant NDIS and MAC providers.
  • Coordination of team discussions and staff allocations to ensure appropriate and efficient care of patients. 
  • Support and promote an environment that fosters staff education and learning, quality improvement and compliance requirements of the service.

Details of appointment

  • Permanent full time, day worker position, working up to 76 hours per fortnight, commencing from as soon as you can join us.

*notwithstanding hours to be negotiated with the successful applicant.

Salary

$118,297 to $125,543 per annum. Our Employer 12% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

In addition, the following allowances calculated on the salaried incremental point may be available:

  • Post Graduate Allowance of up to 10% based on qualification type

  • Remote Professional Development Allowance of $4,062 per annum and an additional salary allowance of 10%

Eligibility

Successful applicants will be required to meet the essential criteria 

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse

  • Current Driver’s Licence
