Must be able to operate Motorized equipment and operate Motor Vehicle with palletized loads when needed. The Diabetes Nurse Educator - Pediatrics assesses, plans, implements, and evaluates the patient centered diabetes education program at the SUNY Upstate Joslin Affiliate. Plans and conducts continuing education program in diabetes for professionals and the general public. Collaborates with other educators and providers in the areas of program development, implementation, marketing and review of clinical programs/services. Participates in the development of education curricula and materials by writing original material and/or reviewing the work of others. Participates in case management, telephonic triage, and patient care planning and goal setting. Provide diabetes education services and general diabetes self-management education (DSME) to patient and families, care partners and community at Joslin and throughout the Upstate Network. |

In this position, the social worker will work directly with patients who may be victims of community violence and their families who present to the hospital, utilizing evidence based trauma informed care. The hospital-based SNUG (Should Never Use Guns) Social Worker will work closely with the Syracuse Community SNUG Site Program and the Division of Criminal Justice Services to improve access to victim services. SNUG follows the Cure Violence Model of violence prevention dedicated to reducing gun violence in high risk neighborhoods and on behavioral changes in the target population to prevent victimization and perpetuation of gun violence. Social Workers provide comprehensive psycho-social assessment, management and support to at risk patients and families. In addition, the social worker addresses social determinants of health and works closely with the multidisciplinary team. The candidate shall gain knowledge of social determinants, population health and drivers of utilization. Duties will include consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patients and families. Candidate must possess communication skills, have knowledge of available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities. |
