Clinical Care Coordinator

At Brotherhood of St. Laurence (BSL), we are seeking a passionate Clinical Care Coordinator to join our dedicated team Residential Care team.

Location: Clifton Hill

Job Type: Permanent Full-Time working 38 hours per week

Salary: $126,293.95 to $124, 507.76 per annum, based on skills and experience plus 12% superannuation. (pro-rata for part-time employment)

About the role

Working in collaboration with the Head of Residential Services or delegate, the Clinical Care Coordinator provides comprehensive and accurate nursing assessments and care plans, and day to day support to clinical and care staff

Key Responsibilities

  • Develop, maintain and continually improve the organisational model of care seeking internal and external information to ensure that best practice is achieved.
  • Develop, monitor and report on Key Performance Indicators (KPI’s) related to care including a range of appropriate clinical indicators. 
  • Develop and continuously improve clinical documentation, tools and reports. 
  • Assist Head of Residential Services to close identified gaps in process, policies and procedures by mentoring, educating and monitoring progress on improvements. 
  • Act as a professional role model and a credible resource by demonstrating and disseminating knowledge and advanced clinical expertise in aged care nursing. 
  • Liaise with and support the activities of the ANACC/Quality Coordinator to ensure revenue is maximised via the relevant funding model. 
  • Provide support and guidance to research projects to ensure that innovations are considered in line with service improvement without denigration of service standards and delivery. • Coordinate high-quality, person-centred care, in accordance with policies and procedures, relevant standards for practice, code of conduct for nurses and relevant legislation including The Aged Care Act and the Quality Standards. 
  • Ensure holistic care and services that incorporate residents’ beliefs, values, needs and culture. 
  • Provide Medication management functions, oversee delegated medication administration and administer medications as required 
  • Promote and directly consult with residents, relatives and others through formal processes daily regarding all aspects of care delivery, including clinical care, ancillary services, lifestyle services and any changes affecting the resident

What we are looking for

We are seeking an enthusiastic and motivated individual who has a passion for supporting the mission of BSL and delivering excellence in your role.

To be successful in this role, we are looking for someone who has the following key skills and experiences:

  •  Registered Nurse with current APHRA registration and significant experience in residential aged care 
  • Relevant experience and knowledge of all aspects of gerontological clinical care as well as understanding and management of aged care quality standards, ANACC and accreditation processes 
  • Demonstrated knowledge and competency in Microsoft Office Programs including Word, Excel and Outlook•
  • Exemplar clinical expertise in aged care •
  • Sound people management skills with the ability to lead multi-site and multifunctional team 
  • Well-developed interpersonal and communication skills with the ability to build effective relationships and liaise across all levels both internally, externally and with people from diverse backgrounds 
  • Proven ability to identify and resolve problems and make appropriate recommendations 
  • Excellent organisational and time management skills with the ability to plan workload, prioritise and meet deadlines
