Transition Coordinator - Permanent fulltime or part-time

 

 

About the Role

The Transition Coordinator for the WREN Inpatient Unit plays a central role in coordinating participant transitions, both when admitted to the hospital and when discharged to their home or next care setting.

This role requires close collaboration with participants, their chosen supports, the multidisciplinary team, and external agencies to create personalised plans that ensure seamless transitions and continuity of care.

Key Responsibilities

  1. Admission Support
  2. Liaison with Multidisciplinary Team (MDT)
  3. Contacting External Agencies
  4. Collaborative Engagement
  5. Documentation of Comprehensive Plans
  6. Ensuring Discharge Summaries and Discharge HoNOS are accurate and complete
  7. 7-Day Follow-Up Phone Calls
  8. Working with Peer Workforce

 

 

About You

Essential

  • Registered Nurse with AHPRA
  • Minimum 2 years’ post grad experience
  • Experience in Mental Health Nursing, preferably in an acute inpatient setting.
  • Strong understanding of mental health conditions, treatment modalities, and recovery principles.
  • Excellent communication and interpersonal skills to engage effectively with patients, families, and a diverse MDT.
  • Ability to work collaboratively in a fast-paced, team-oriented environment.
  • Proficiency in electronic medical records systems and documentation practices.

Desirable

  • Experience working with women experiencing mental health issues
  • Proficiency in electronic medical records
  • Postgraduate qualifications or training in mental health nursing

 

 

Remuneration

Per Hour - 38.56 + superannuation based on experience level and qualifications, exclusive of penalties or allowances. (RN year 3)

Requirements

  • Must provide a National Police check conducted within the previous 12 months
