About the Role
Caboolture Private Hospital is seeking a skilled Discharge Planner, the role would be a Level 2 Registered Nurse (Clinical Nurse) to join our team, working closely with the Medical/Surgical Ward Nurse Unit Manager (NUM) and their team. This vital role involves coordinating and facilitating smooth patient discharges, ensuring that patients receive appropriate follow-up care and resources.
About You
As a Discharge Planner, you will:
- Collaborate with medical and nursing staff to develop comprehensive discharge plans.
- Ensure timely, safe, and efficient patient discharges.
- Communicate effectively with patients, families, and healthcare providers to arrange follow-up care and services.

Remuneration
On offer is base hourly rate ranging from 52.86 - $56.83+ superannuation based on experience level and qualifications, exclusive of penalties or allowances. (QLD Reg Nurse EA Level 2 Grade 1-4 )

This hospital has 43 beds, 3 fully equipped operating theatres, a 12 chair Day Procedure Unit and a comfortable day oncology unit. Requirements
- Must provide a National Police check conducted within the previous 12 months
