Position Introduction

This position serves as a case manager for home and community based waiver programs through the case management department at the College of Nursing. This position will carry a full time caseload.

Required Education and Experience

• Bachelor's degree in Social Work, Psychology, Sociology, or other Health or Human Services related discipline.
• 24 months experience in home and community-based services, case management, or related healthcare setting.

Required Certifications and Licenses

Certified Case Manager (CCM). The employee is allowed 1 year from hire date or promotion to Senior Case Manager to obtain the CCM certification.

Department Preferences

• One year of paid professional experience in direct service provision, care planning, and has provided services to the aging and/or disabled population.
• Certified in Case Management
• Experience in a home-like setting
