Success in this position requires someone who enjoys coaching others and building high-performing teams. You will play an integral role in recruiting, onboarding, training, and mentoring new Case Managers while providing ongoing guidance that encourages professional development and clinical excellence. Your leadership will directly influence staff engagement, service quality, and program effectiveness. As an experienced clinician, you'll also have the opportunity to improve systems and processes that enhance care delivery. Through quality improvement initiatives, performance monitoring, internal audits, and collaboration with organizational leadership, you'll help ensure compliance with state, federal, and contractual requirements while identifying opportunities to strengthen operations and improve member outcomes. Required Education and Experience:

  • Bachelor's Degree in Nursing for Licensed Registered Nurse or Master's of Social Work with an LCSW.
  • 24 months experience in home or community-based services, case management, or a related healthcare environment.

Required Certifications and Licenses:

  • Licensed Registered Nurse or Licensed Clinical Social Worker (LCSW) Certified Case Manager (CCM) or ability to obtain one of the aforementioned licenses within 6 months of hire date.

Department Preferences:

  • College of Nursing Case Management requirements: 12 months paid professional experience performing duties which encompass the core functions of case management: assessment, planning, implementation, monitoring and reassessment.
  • One year paid experience as an ADvantage case manager as well as one year paid experience as a manager of personnel.
  • Master's Degree in Nursing, Social Work, Healthcare Administration, Public Health, or related field preferred.
  • Current professional licensure (RN, LCSW, LPC, or other applicable healthcare credential) preferred
  • 24 months management experience including hiring, training, assigning work, evaluating performance, commendation and/or disciplinary action and termination proceedings.
  • Proven problem-solving skills, including the ability to resolve issues using critical thinking and appropriate decision-making.
  • Excellent organizational, problem-solving, and critical thinking skills, with the ability to prioritize work, meet deadlines, and recommend process or quality improvements.
  • Ability to work independently while exercising discretion, confidentiality, and professional judgment, and seeking guidance as appropriate.
  • Excellent Communication and Collaborative Leadership Skills: Ability to lead teams, facilitate meetings, work effectively with interdisciplinary partners, and contribute to department and organization-wide initiatives.
