Position Introduction: Are you a strategic healthcare leader who is passionate about improving lives through innovative, community-focused care? We are seeking an experienced and collaborative leader to oversee the operational and clinical success of our community-based case management programs. As the Director of Community-Based Case Management Programs, you will serve as a key member of the leadership team, partnering closely with the Executive Director to shape the strategic direction of the department while ensuring the successful day-to-day operation of multiple revenue-generating clinical programs. This position offers the unique opportunity to balance strategic planning with hands-on operational leadership, helping to advance programs that directly improve access to quality healthcare services within the communities we serve. Success in this role requires a leader who is equally comfortable evaluating operational performance, interpreting data, improving business processes, and implementing strategic initiatives that enhance quality, compliance, and financial sustainability. You will help ensure programs consistently meet regulatory requirements while identifying opportunities to improve efficiency, strengthen service delivery, and achieve organizational goals. Your leadership will directly influence the quality of care provided to members, the engagement of staff, and the continued growth of the department. We are looking for a professional with a strong background in healthcare operations, clinical program management, and employee leadership who enjoys solving complex challenges and leading organizational change. The successful candidate demonstrates sound judgment, excellent communication skills, and the ability to make informed decisions in a fast-paced environment while maintaining a strong commitment to integrity, collaboration, and continuous improvement. Candidates must possess a bachelor's degree in Nursing, Public Health Administration, or another health-related field (or an approved education and experience equivalent) and have at least two years of management experience in a healthcare setting. A master's degree in Social Work, Nursing, Public Health, Healthcare Administration, or a related health field is strongly preferred. Registered Nurses must maintain an active Oklahoma RN license, and certain programs may require additional certifications based on operational needs. Required Education and Experience:
Equivalency/Substitution: Associate's Degree in Nursing or other health-related field and 24 months experience in health care or related experience in lieu of the Bachelor's Degree in Nursing. All other degree fields experience or a combination of education & related experience can be considered in lieu of degree.
