Position Introduction:

Join our team as a Lead Case Manager, overseeing the case management process from assessment, reassessment, service planning, monitoring while ensuring high-quality, consumer-centered services. This role provides leadership to case management staff, supports quality improvement initiatives, coordinates care and services, and helps drive overall program success.

Required Education and Experience:

  • Bachelor's degree in Social Work, Psychology, Sociology, or other Health or Human Services related field.
  • 24 months experience in case management.

Equivalency/Substitution: Will accept an Associate's degree in Nursing in lieu of the Bachelor's degree.

Required Certifications and Licenses:

  • Social Work (LCSW), Master's Prepared OR Registered Nurse (RN).
  • Certified Case Manager (CCM) (allowed 1 year from hire date to obtain the CCM certification)

  • Bilingual - English/Spanish
