Position Summary

Candidate with a Licensed Master of Social Work (LMSW) is sought for a part-time position (8-10 hours a week) within the Department of Neurology’s Muscular Dystrophy Association Clinic.

Responsibilities

• Provide social work support for pediatric patients and adult patients who are cared for by the Muscular Dystrophy Association Clinic
• Complete forms required for referrals including homecare, in-home rehab services, respite, transportation, palliative care and hospice services, M11Q forms, durable equipment forms, and any other forms that can be completed within the social worker’s scope of practice.
• Compose letters of medical necessity.
• Provide case management, crisis intervention, and supportive counseling services to the individuals and families seen within the MDA clinic.
• Provide referrals and resources to patients and caregivers.
• Participates in staff meetings, in-services, administrative activities, and community outreach events.

Minimum Qualifications

• Must hold a master’s degree in social work, and 2 years of related experience.
• Must hold a New York State LMSW or LCSW license.
• Certificate from the New York State Education Department. 

Preferred Qualifications

• Bilingual in English/Spanish strongly preferred. 

Previous experience working with this population, or as a medical social worker.
