Country Health Connect is seeking a compassionate and dedicated Home Support Worker to join our team in Millicent. As a Home Support Worker, you will play a crucial role in providing essential cleaning and shopping services to clients of Domiciliary Care. Your efforts will directly contribute to promoting independence and enhancing the quality of life for our clients, while preventing premature or inappropriate admissions to residential care facilities or hospitals.

Key Responsibilities:

• Deliver timely and efficient cleaning and shopping services to clients in their homes.
• Assist clients with household tasks to maintain a safe and comfortable living environment.
• Promote independence and wellbeing through respectful and person-centered care practices.
• Collaborate with healthcare professionals to support holistic care plans for clients.

Country Health Connect focuses on the delivery of exceptional care to those living within our communities who require some support to remain living independently or reside in one of our residential aged care facilities.

Our professional and caring staff deliver health care and wellbeing support across regional South Australia in hospitals, local health services, community centres, residential aged care facilities and clients’ own homes.
