About the role

Join our dedicated team supporting regional South Australia by delivering expert nursing care to clients with chronic conditions through innovative virtual care. We offer an opportunity to reduce hospital admissions, improve patient journeys, and collaborate with multidisciplinary teams while harnessing digital technologies - all within a flexible, supportive rural health network.

How will you contribute

  • Monitor and support clients daily via the Virtual Clinical Care service, providing expert advice and case management.
  • Deliver evidence-based self-management education and clinical guidance to clients with complex chronic conditions.
  • Collaborate with multidisciplinary teams to coordinate seamless, patient-centred care across regional health services.
  • Assist with system improvements, data management, and virtual service delivery innovations to enhance client outcomes.

  • Are you confident working autonomously in a virtual environment, managing complex workloads under pressure?
  • Can you communicate effectively and respectfully, including engaging with Aboriginal communities in a culturally appropriate way?
  • Are you experienced in using digital health technologies and passionate about supporting clients with chronic conditions to self-manage their health?

The RSS is a business unit in SA Health and provides services throughout regional South Australia to support the six regional local health networks (LHNs). It employs around 260 people who work in 37 regional, rural and remote SA locations, or in the RSS’s three Adelaide offices. Applicant Information

This is a part time vacancy, working 45 hours per fortnight and salary will be adjusted according to actual hours worked.
