Senior Social Worker - Community Geriatrics | Make a Real Difference

Join our expanding Community Geriatrics team in this rewarding, ongoing part‑time role supporting older people across Adelaide’s southern region. As a Senior Social Worker, you will deliver specialised case management to older community‑dwelling clients, working closely with their families and support networks to maximise their supports and ensure holistic, person‑centred care.

About the role: 

The Senior Social Worker will work as a Case Manager within this Geriatrician led service. Reporting operationally to the Nurse Unit Manager, this role is responsible for providing assessment and interventions for older people with cognitive decline, often with complex medical, functional and/or psychosocial needs.

Duties for the role will include:

  • Drive the delivery of high quality and safe care consistent with the SALHN Integrated Governance Framework and Consumer Engagement Framework and Plan,
  • Independently assess client’s abilities, limitations and needs using a variety of formal and informal assessments,
  • Design and implement evidence-based intervention programs tailored to diverse patient populations,
  • Drive interdisciplinary clinical and strategic planning initiatives to enhance the overall service provided to clients,
  • Documenting assessment findings and interventions in accordance with service standards,
  • Providing specialised client and carer education about their level of functioning and ongoing care recommendations, including advocacy for their needs,
  • Mentoring and supporting other members of the team,
  • Providing an expert consultative service

About you:

  • A degree or equivalent in Social Work, entitling membership of the Australian Association of Social Workers,
  • Demonstrated high level competency and extensive knowledge in Social Work assessment and therapeutic interventions to clients with complex needs with a broad range of Senior Social Worker clinical skills.
  • Ability to critically analyse or evaluate issues related to clinical care in social work in a manner which can be applied to quality activities, service evaluation or research.
  • Demonstrated ability to establish and maintain effective networks and partnerships with a broad range of culturally diverse clients and stakeholders, including patients and their families, community services / agencies and other service providers, within an interdisciplinary setting.
  • Demonstrated high level written communication skills including the ability to prepare concise, accurate and timely clinical reports and data.
  • Experience providing social work services to older people in a community programme,
  • Experience as a social worker in a health setting,
  • Experience providing care planning, liaison and case management,
  • Experience in the training of students and less experienced staff,
  • Experience in program evaluation, service development, combined with a broad base of experience in Senior Social Work practice
