SNUG follows the Cure Violence Model of violence prevention dedicated to reducing gun violence in high risk neighborhoods and on behavioral changes in the target population to prevent victimization and perpetuation of gun violence. Social Workers provide comprehensive psycho-social assessment, management and support to at risk patients and families. In addition, the social worker addresses social determinants of health and works closely with the multidisciplinary team. The candidate shall gain knowledge of social determinants, population health and drivers of utilization. Duties will include consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patients and families. Candidate must possess communication skills, have knowledge of available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.
