About the role
As a Registered Nurse (RN1) within Hampstead Rehabilitation Centre’s Care of the Older Person and Community Transition (COACT) and Sub-Acute Geriatrics teams, you’ll deliver evidence-based, person-centred care to older adults as they transition through complex stages of their journey. You’ll use your clinical judgement to lead care planning, support team coordination and contribute to a safe, connected and therapeutic environment.

Key Responsibilities

  • Complete comprehensive assessments and develop individualised care plans
  • Deliver safe, evidence-based nursing care and evaluate patient outcomes
  • Recognise clinical deterioration and initiate timely escalation
  • Provide leadership on shift, supporting ENs, students and less experienced staff
  • Collaborate closely with multidisciplinary teams, patients and families
  • Contribute to quality improvement, patient safety and service development

About you
You’re a compassionate and capable Registered Nurse who brings sound clinical judgement, strong communication skills and a genuine commitment to person-centred care. You thrive in a multidisciplinary team environment, stay composed under pressure and use your problem-solving skills to deliver safe, high-quality care for older adults with complex needs.

  • Registration (or eligibility) with the Nursing & Midwifery Board of Australia
  • Well-developed assessment, critical thinking and care planning skills
  • The ability to prioritise workload, work flexibly and respond confidently to change
  • Effective communication, negotiation and conflict-resolution skills
  • A collaborative approach that supports positive relationships across the multidisciplinary team
  • Commitment to continuous learning, professional standards and high-quality, consumer-centred care n-centred practice, safety and continuous improvement
