About the Role

Lead innovative out of hospital care that helps people receive the right care in the right place. You'll shape hospital avoidance strategies, support safe discharge pathways and build strong partnerships that improve patient flow and outcomes across CALHN.

• Lead hospital avoidance and supported discharge initiatives
• Build rapid assessment pathways across care settings
• Coordinate safe, timely discharge for complex patients
• Partner with health and community services
• Guide best practice, education and service improvement

About You

You're a collaborative nursing leader who thrives in complex environments and is passionate about improving patient care beyond the hospital.

• Current AHPRA nursing registration and practising certificate
• Expertise in triage, assessment and clinical decisions
• Experience leading change and guiding multidisciplinary teams
• Strong knowledge of NSQHS Standards and relevant legislation
• Skilled in hospital avoidance and community care pathways
